MCA Care Providers Corp ("we," "us"), 12929 SW 230th St, Miami, FL 33170, provides non-medical in-home care and operates mcacareproviders.us. We treat information about clients' health and daily needs as confidential. This policy explains what we collect and how we protect it.
Information we collect
- Inquiries: name, contact details, relationship to the person needing care, ZIP code, timing, and your message.
- Client records: care plan, routines and preferences, emergency contacts, physician and pharmacy contacts, relevant health information needed to provide safe care, caregiver visit notes, and billing records.
- Caregiver applicants: application details and background screening results, handled as required by Florida law.
- Website: basic server logs. The care needs assessment runs in your browser, and we don't receive your answers.
How we use it
- To plan, schedule, provide, and supervise care.
- To coordinate with physicians, home health agencies, and others involved in care, with consent where required.
- To bill clients or their long-term care insurers, and to keep required records.
- To respond to concerns and improve quality.
Sharing
We don't sell personal information. We share client information only with caregivers assigned to the client, with family members or representatives the client has authorized, with healthcare providers involved in care, with insurers for billing, and as required by law, for example to report suspected abuse or neglect or in an emergency.
Security
Client records are kept in secure systems with access limited to staff who need them. Caregivers are trained in confidentiality. No method of storage is perfectly secure, and we'll notify affected individuals of a data breach as required by Florida law.
Your rights
Clients and their authorized representatives may request access to, or correction of, client records by emailing support@mcacareproviders.us. Some records must be retained for legal reasons.
Changes
We may update this policy and will revise the effective date when we do.
