Hospital to home
The first days and weeks after discharge can be the hardest. Extra hands at home make recovery safer and less stressful for everyone.

Follow-ups & routines
Rides to follow-up appointments and support between visits from home health nurses or therapists.
Step down
Hours reduced as strength returns, or adjusted into an ongoing care plan if needed.
Questions
Can you start the day of discharge?
Often, yes, especially if you call as soon as the discharge date is known. The hospital's case manager can also help coordinate timing.
Does Medicare's home health benefit cover this?
Medicare home health covers intermittent skilled care ordered by a doctor, provided by a certified home health agency. Our non-medical help is separate and is usually private pay or long-term care insurance. Many clients use both together.
